It could have something going on inside if its losing weight, I know that alot autumn babies get "lungworm" that can kill them.
One of mine had something going on but they put it on treatment and thankfully it's okay now, I think the little guy's probably got something going on hopefully you can get it to a rescue and its able to be treated.

I have seen that hedgehogs make two nests sometimes, This is to give them a different option incase of the nests is disturbed or floored with rain etc.
He / She is probably doing this which is a good sign because both nests are in your garden!
