No, but you could! Although widening existing holes with a hole saw can be a bit tricky (not impossible).
General recommendation seems to be 45mm, yes, and I think somewhat bigger inside than a typical bluetit box (14cm+). I wish I was telling you that from experience, but my starling boxes are sitting sadly unused!
